Please use a separate form for each move. Enter the Equipment ID, located on the Ricoh sticker on the front of the machine and the model number. You need to supply a site contact for both pick up and delivery locations. If you are the site contact for either or both and have already entered you information, you can just put "self" as contact.
Please enter a relocation date range. Ideally we would like at least two weeks notice.
